The female passenger trapped inside the red Ford Fiesta has been removed by emergency services personnel.
The passenger was assisted by ambulance officers and firefighters onto a spinal injury board. She was then conveyed to Port Macquarie Base Hospital.
Witness Jarrad Lawless saw the accident unfold as he prepared to turn onto Lake Road.
"The P-plater came up Blackbutt, tried to turn right onto Lake Road and I think they didn't see the bigger car coming up Lake Road. That's when the P-plater got hit," he said.
Mr Lawless said he saw the driver of the other car, a white Toyota Prado, get out of the car after the accident.
Highway Patrol and other police officers are currently directing traffic on Lake Road with the assistance of Fire and Rescue NSW personnel.

Ambulance officers are attending to the occupants of two vehicles involved in an accident on Lake Road.
The accident occurred at approximately 11.40pm today and has blocked the two inside lanes of Lake Road outside Amber Tiles at the intersection with Blackbutt Road.
Ambulance and fire rescue are in attendance.
Traffic is moving slowly around the scene in the outside lanes.
NRMA tow trucks have arrived to remove the vehicles from the scene.
